Health Outcomes Survey-Modified (HOS-M) Survey Vendors

The Centers for Medicare & Medicaid Services (CMS) developed the HOS-M Vendor Program to establish standardized data collection to promote comparability of results across Programs of All Inclusive Care for the Elderly (PACE) organizations and Medicare Advantage Organizations (MAO). PACE organizations and MAOs select and contract with a CMS-approved HOS-M survey vendor to administer the HOS-M survey for PACE or Fully Integrated Dual Eligible (FIDE) Special Needs Plans (SNP) for frailty using a mixed mode (mail and telephone) protocol. To become a CMS-approved HOS-M survey vendor, an organization must demonstrate that it has the capability, experience, and personnel to collect and report accurate HOS-M survey results and successfully complete the annual HOS-M Survey Vendor training in April 2024.

This table lists the survey vendors that are conditionally-approved to administer the HOS-M in 2024, including contact information for the organization’s primary contact. Conferring final approval status is contingent on survey vendors successfully completing HOS-M survey vendor training in April 2024.
