2026 HOS Survey Vendors

The Centers for Medicare & Medicaid Services (CMS) developed the Medicare Health Outcomes Survey (HOS) Vendor Program to establish standardized data collection to promote comparability of results across Medicare Advantage Organizations (MAOs). MAOs contract with a CMS-approved HOS survey vendor to administer the survey using a mixed mode (mail and telephone) protocol. To become a CMS-approved HOS survey vendor, an organization must demonstrate that it has the capability, experience, and personnel to collect and report accurate HOS survey results. Prior to becoming a CMS-approved survey vendor, organizations must apply, be invited to, and successfully complete survey vendor training.

Survey vendors administer the HOS using the HEDIS protocol described in HEDIS Volume 6 and adhere to all guidelines in the 2025 Medicare Health Outcomes Survey Quality Assurance Guidelines and Technical Specifications (QAG), including guidelines for protecting member confidentiality and requirements for protocol adherence. This table lists the survey vendors that are invited to attend the HOS survey vendor training in 2026, including contact information for the organization's primary contact. The approved survey vendors who successfully complete HOS survey vendor training will be available in March 2026.
